Online Admission Form for Schools: Fields and Steps
By Sajre Edutech team, Dharwad ·
TL;DR: A good online admission form is short. It asks for the details the office needs to admit the child and run the school year, and nothing more. Parents type their own details once, the office checks the original documents at the desk, and the student's record is ready before the first day of class. This guide lists the fields, the documents and the steps.
This guide is general information for school offices. Admission rules (age limits, reserved seats, required documents) are set by your board and your state. Follow their current rules.
Why move the admission form online
The paper admission form has a hidden second step: someone in the office types every form into a register or a spreadsheet. That is where names get misspelt, phone numbers lose a digit and the same family ends up written three different ways.
An online form moves the typing to the person who knows the details: the parent. The office's job becomes checking, not copying. When admissions are busy, that difference adds up.
The fields to ask for
Ask only for what you will use. Every extra field is one more reason for a parent to stop halfway.
| Section | Fields | Why the school needs it |
|---|---|---|
| Child | Full name (as on certificates), date of birth, gender | Certificates, receipts and age eligibility |
| Admission | Class applied for, academic year, branch (if you have more than one) | Puts the application in the right place |
| Parents or guardian | Father's, mother's or guardian's name, relationship | Records and certificates |
| Contact | One mobile number the parent actually uses, email (optional), address | Fee reminders, parent login, emergencies |
| Previous school | School name, board, last class passed | Students joining from another school |
| Other | Anything your board or state requires (for example category, if a reserved-seat scheme applies) | Compliance, only where it applies |
Two fields deserve extra care:
- The child's name. Ask for it exactly as it should appear on the transfer certificate and the fee receipt. Fixing a spelling later means fixing it in every record.
- The parent's mobile number. Most school software uses it for the parent's login and for messages. Ask the parent to confirm it at the desk.
The documents to check at the desk
Many schools keep the online form for details only and check original documents once, in person. Common documents are:
- birth certificate (proof of age);
- transfer certificate and last report card, for students coming from another school;
- address proof;
- passport-size photographs.
On Aadhaar: a school may ask for it, but the Supreme Court's September 2018 judgment in K.S. Puttaswamy v. Union of India said it cannot be made compulsory for school admission. Accept other proof and do not turn a child away for not having it.
Collect only the documents you need, and store scanned copies only if you have a reason to. A child's records are personal data, so keep them where only the people who need them can see them.
A simple admission process, step by step
1. Open admissions. Decide the classes, seats and the last date. Put the form link on your website, your WhatsApp status and the notice at the gate.
2. Parents fill the form. The parent fills in the details on their phone. The application reaches the school office.
3. The office reviews. Check the class and age against your rules, and call the family if something is missing.
4. The family visits the desk. The office checks the original documents, confirms the mobile number and the spelling of the child's name, and confirms the class and section.
5. Fee and admission number. The family pays the admission fee and gets a receipt. The student gets an admission number that is never reused. Our guide on school fee receipt format lists what the receipt should show.
6. The record is ready. The student now has one record that attendance, fees and the timetable can use from the first day.
Common mistakes
- A long form. Three pages of questions on a phone screen lose parents. Ask only what you use.
- Two lists. If applications sit in one place and admitted students in another, someone ends up retyping. Keep one record per student from the first application.
- No check at the desk. An online form is typed by the parent. A two-minute check of name, date of birth and mobile number saves corrections all year.
- Forgetting existing students. The online form is for new families. Your current students still need to come into the same system. Our guide How to import student data from Excel explains how to prepare that list.
How this works in EduTechMonitor
EduTechMonitor, the school management software made by Sajre Edutech in Dharwad, Karnataka, has an online admission form. New families fill it in, and the details go into the school's student records, so the office reviews them instead of retyping them. Existing students are brought in from a spreadsheet; our team does the first import together with your office during setup.
Each student then has one record used for daily attendance, fee collection with printable receipts, the class timetable and the parent's login. Parents log in with a one-time password sent by SMS to their registered mobile number. A trust with several schools or branches keeps each branch's admissions under its own admin.
What is not available yet: online payment of admission or application fees. Fees are paid at the school, and the office records them and prints a receipt.

Frequently asked questions
What fields should a school online admission form have?
Keep it to what the office needs to admit the child and run the year: the child's full name as it should appear on certificates, date of birth, gender, class applied for, both parents' or the guardian's names, a mobile number the parent actually uses, address, and the previous school and last class passed if any. Ask for anything else only if you will use it.
Which documents do schools usually check at admission?
Commonly a birth certificate for proof of age, the previous school's transfer certificate and last report card for students moving from another school, address proof, and passport-size photographs. Your board and state may require more for particular cases, so follow their current rules.
Can a school make Aadhaar compulsory for admission?
No. In its September 2018 Aadhaar judgment (K.S. Puttaswamy v. Union of India), the Supreme Court said Aadhaar cannot be made compulsory for school admission. A school may ask for it, but should accept other proof and should not refuse a child for not having it.
Should parents upload documents on the online form?
It is optional. Many schools let the form collect only the details, and check the original documents once at the admission desk. That keeps the form short, avoids storing many scanned copies, and the office still sees the originals.
Does an online admission form replace the admission desk?
No. The form replaces retyping. The office still meets the family, checks the documents, confirms the class, collects the fee and gives the admission number. The difference is that the details are already typed in, by the parent, before they arrive.
Does EduTechMonitor have an online admission form?
Yes. In EduTechMonitor, the school management software made by Sajre Edutech, new families can fill an online admission form and the details go into the school's student records. Online payment of admission fees is not available yet; fees are paid at the school, and the office records them and prints a receipt.
